My experience with Personal Health Records began in 2003. As a full-time caregiver for my mother-in-law and a part-time caregiver for my mother, I found myself struggling to keep track of my mother-in-law’s ever-increasing medication list. Doctor’s appointments were an ordeal, and left me searching for a better way to ensure proper care.
A comprehensive PHR was the only way to combat her memory loss and fulfill my need for clarity. A text document on the computer, printed for reference, has grown to a comprehensive list of contact information, allergies, vaccinations, supplements and 19 medications. While admittedly low-tech, the two PHRs I created are both complete and understandable. I list each drug’s name, its purpose and the dose on the PHR I share with medical professionals. My “at-home” PHR features medication listed by time of day so that anyone can easily step in and ensure my mother or mother-in-law receives the proper medication at the proper time.
PHRs have been an enormous asset to my duties as a caregiver, simplifying routine medical appointments such as medication adjustments. After all, there’s no guessing when it’s on a piece of paper. Now, I’m encouraging my family members to gather their personal health information before they need it. PHRs truly represent the best of both worlds, simplifying and potentially saving your life or those you care for.
