This personal health record is designed to help you keep your family member's medical information organized in one place. It includes space for you to write in information about your family member's medical conditions and treatments. It also includes space to write down information about insurance coverage and advance directives. Make sure to keep this information up-to-date and in an easy-to-find place. It is a good idea to keep this information together with your family member's list of medications. You can use the Medication Management Form for that and keep these two items together.
